Hospitalists are extremely familiar with hospital procedures and staff working in all departments. Because hospitalists only work inside the hospital, they can offer around-the-clock, in-house care and directly respond to patient needs. In case of emergencies or changes in a patient's condition, a physician will be close by to respond quickly.
Patients will have their own teams of hospital-based providers, who will see them on a daily basis, modify their medication regimens if necessary, and order appropriate tests or procedures. If a consultation with another specialist is required, this will be arranged by the team.
We will also keep the patient's primary care physician updated as to his/her status. Upon discharge, patient’s medical records will be available to the primary care physician to aid in the continuum of care. This way, any follow up care needed, from questions to prescription refills, can be handled by the personal care physician.
