Patient Information:
At Rectal and Colon Surgery we aim to see our patients as quickly and efficiently as possible, with minimal time spent in the waiting room. For this reason we ask our patients who have not been to the office in more than six months to arrive 30 minutes before their scheduled appointment to fill out all necessary registration and medical history updates.
As an added convenience, you may download the entire packet of registration forms and complete them prior to arriving for your appointment and check in with your completed forms 15 minutes prior to your scheduled appointment time.
Please be advised that if you do not arrive at our office in time to complete these forms prior to your scheduled appointment it may be necessary to reschedule your appointment for another day. Please be considerate to our other patients by complying with these guidelines.
Your First Appointment
Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Patient's insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records
- Completed Patient medical history form (PDF)
Insurance and Payment Information
Rectal and Colon Surgery is a provider for Medicare and most major insurance plans, and provides insurance billing. You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please contact us. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
Privacy Practices
Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients, and a copy is available upon request.
